We understand that your time is precious and that you need an optometrist who is professional and concise, yet convenient for your lifestyle. In order to make the transition to our office smoother for you, we have created a New Patient Center with everything you need to get started.
In an effort to make you more comfortable when you arrive at our office and to streamline the new patient process, you will be able to complete all the necessary items prior to your first visit. If you are unable to fill out the necessary paperwork, please arrive 15 minutes early prior to your appointment so that you will have time to do so then. At North Shore Eye Care, we strive to make sure every moment we spend with you during your visit is quality time.

Flexible Spending Accounts (FSA) and Health Savings Accounts (HSA)
Many employers are offering FSA and HSA options to employees. Sometimes these are referred to as cafeteria plans and are elective, supplemental insurance saving plans. These plans are designed to let you save money in an account, pre-tax, to pay for additional medical expenses such as eye exams, glasses, contacts and often laser vision surgery. Check with the benefits administrator at your work to see if you are eligible for this program. FSAs and HSAs can typically be used in conjunction with any insurance plan to offset any out-of-pockets costs on your behalf.
